Fill out 1 per Infant/Toddler and Preschooler. This form is used to detrermine whether your child received Free, Reduced or Paid status for meal reimbursement from USDA.
FOOD PROGRAM FORM
Fill out 1 per family. This form is used by USDA to determine how much the center will be reimbusred for each nutritious meal it serves.
